A Legacy of Care, A Future of Opportunity!We are seeking a dedicated and compassionate Physical Therapist to join our team in a skilled nursing facility.RESPONSIBILITIES:Conduct comprehensive evaluations of patients to identify physical, functional, and rehabilitation needs.Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.Monitored and documented patient progress and adjusted treatment plans as necessary.Educate patients and caregivers on exercises and techniques to promote recovery and prevent future injury.Provide hands-on therapy, including exercises, manual therapy, and modalities as appropriate.Ensure compliance with regulatory standards and company policies in all aspects of care delivery.Participate in clinical meetings and contribute to a culture of continuous improvement.QUALIFICATIONS:New passer/graduates are welcomeWith Skilled Nursing or Long-Term Care, experience is a plusPrevious experience as a Physical Therapist (PT) is a plusPhysical Therapy Degree from an accredited programState licensure or eligibility for licensure as a physical therapistIs in good standing with all regulatory agencies and licensing boards as requiredInterest in working with the geriatric populationAbility to work effectively both independently and within a teamWHAT WE OFFER: We value our team members and provide a supportive and enriching work environment.
