Working under limited direction, review patient charts, treatment
plans, and monitor patient outcomes. Through tracking and reporting
the utilization of evidence-based best practices and collaborating
with critical care team to increase/improve the application of
evidence-based clinical practices, the will impact patient
outcomes. The Nurse is responsible for the coordinating, monitoring
and supporting performance improvement services. Works closely with
patient care professionals, including but not limited to physicians
and bedside nurses to meet compliance with recommended
evidence-based practice guidelines.Responsibilities
- Develop systems to identify specific in-patient populations,
related to evidence-based guidelines
- Identify opportunities for improvements based on evidence based
practices, regulatory and accrediting agency requirements.
- Analyze data and identify trends/gaps in high risk, high volume
and/or problem prone processes.
- Responsible for key regulatory reporting to the critical care
team, through chart abstraction/review, i.e. Core Measures, MU
measures, restraints, and other indicators.
- Create tools to track and analyze trends on specific patients'
- Design and implement new processes and clinical care, based on
evidence, to continually improve patient care and outcomes and to
achieve performance targets.
- Communicate findings and provide feedback to the critical care
team regarding compliance with the recommended evidence-based
practices and results of outcomes measures.
- Conduct and report results of clinical outcomes and/or process
monitoring; designs and develops dashboards and other innovative
data visualization with the goal of improving care.
- Advise on the appropriate use of quality improvement tools and
methodologies, such as PDCA, Lean, Six Sigma, and Statistical
Process Control Analysis.
- Apply multiple performance improvement methodologies and tools
to projects (e.g. Lean, Six Sigma, FMEA).
- May mentor and lead the work of others, as assigned.
RN degree from a program of nursing; Bachelor's or Master's degree
in Nursing from an accredited college or university preferred.
Five (5) years' experience in critical care, acute care, hospital
or similar settings.
Demonstrates leadership and effective oral and written
communication skills. Must be innovative and self-directed.
Must possess high level complex communication skills and excellent
computer skills (i.e., PowerPoint, Excel).
Must demonstrate the ability to work well with patients, families,
nursing, ancillary staff and physicians.
Knowledge of performance improvement methodologies (e.g. Lean, Six
Experience with quality monitoring and abstracting data from
medical records required.
Ability to develop a detailed understanding of Core Measures and
Meaningful Use Quality Measures definitions is required.
Existing knowledge of Core Measures is preferred.
Familiarity with EPIC preferred.
Valid California RN License
