Hours80Work ShiftDay: 8 hoursJob DescriptionForm, lead, and
facilitate cross functional teams in the planning, developing,
coordinating and implementing of the proactive consultation model
with the goal of early intervention in situations of increased
complexity to improve efficiency, quality, and the patient
experience. Facilitate patient and healthcare team experience and
performance improvement teams in collaboration with clinical and
non-clinical team leaders and senior leaders.The consultation
liaison (C-L) program manager serves as the clinical leader for the
consultation-liaison service, advancing the care of the patients
throughout the organization who require psychiatric consultation.
The role leads performance improvement initiatives through
application of clinical expertise, knowledge of both clinical and
administrative settings and frequent interactions with a variety of
clinical and non-clinical roles. The program manager will utilize
their expertise in enhancing the program, develop evidenced-based
practice, education and collaboration, mentoring, and demonstrate
change leadership. -The program manager coordinates the team's
efforts to design, implement, measure, and report on early
psychiatric intervention strategies in response to consultation
requests across the continuum of care. - The program manager
conducts comprehensive patient studies to evaluate activities and
outcomes, manages policies and protocols, and reviews current best
practice through networking and ongoing education.Key
Responsibilities
- Collaborates with the MHAS leadership team to improve the use
of evidence-based practice for the C-L service and improve clinical
outcomes by co-leading PI projects, developing education modules,
and gap analysis. - (Examples: identifies barriers to be addressed,
suggests creative actions and education plans, and maintains senior
leader support.)
- Leads, facilitates and collaborates with the multi-disciplinary
team across the enterprise to integrate evidence-based practice
recommendations into clinical practice and organizational
processes. - Works directly with service line leaders, medical
staff (e.g. Co-Management), nursing and community providers to
improve care for the patients requiring psychiatric consultation.
(Examples: develops new processes, forms, clinical tools to advance
care.)
- Maintains current knowledge of management of the C-L program,
and of current clinical practice /other guidelines and regulatory
specifications to include Joint Commission as well as health plan
specialty certifications. - (Examples: reads and accesses
resources, shares new developments, attends workshops and
conferences.)
- Supervises and controls all program-specific data generation
for C-L programs including IT/iCare reporting, patient-reported
outcomes, and data registries. Partners with clinical data
analysts/staff. Reviews and analyzes data regarding clinical
outcomes and regulatory compliance. - Provides data and reports to
executive and physician leadership, and department directors. -
Works closely with clinical analysts and IT/iCare staff to develop
reports and improve data collection. (Examples: length of stay,
re-hospitalization and other pertinent data.)
- Concurrently monitors the program's performance of care on C-L
service across the enterprise including med/surg units, ED in both
MV and LG locations, providing education and intervention with
clinical staff and providers as necessary to improve delivery of
care.
- Provides feedback to the team and develops strategies to
overcome organizational barriers regarding knowledge, process and
interdepartmental collaboration. Additionally, brings frontline
hospital staff feedback to the multidisciplinary team. -
- Demonstrates teamwork and utilizes communication/ influencing
expertise to foster collaboration among the health teams and in the
adoption of best practices. - (i.e., supports organizational
objectives, upholds ethical and moral principles in delivering
compassionate care, and contributes to the greater good of the
organization.).
- Complies with regulatory standards and laws and strives to
continue to advance the program. (i.e., maintains knowledge of
current, changing and proposed standards and implements them in the
service, reviews policies and procedures periodically and revises
if necessary, holds staff accountable for meeting standards with
regard to safety, licensure, required health screenings and
mandatory education, defines Corporate Compliance Plan verbally and
in writing as it applies to MHAS).
- Demonstrates complex/high-level communication skills. Presents
to Executive Leadership. - (i.e., interprets "Big Picture" for
department and communicates it to staff and other departments,
positively influences paradigm shifts, holds effective meetings,
holds monthly staff meetings, gives clear presentations, excels at
both verbal and written communication.)
- Plan and directs services to improve clinical outcomes. (i.e.,
plans and manages workflow to meet patient and healthcare team
needs, develops operational plans, develops policies and
procedures, determines proper resource allocation and
organizational structure, coordinates activities with others and
communicates plans, trains and educates staff.) - - -
-QualificationsBachelor's degree in nursing or related field;
Master's preferred. Three (3) years' clinical experience leading
teams in the specialty. Knowledge of inpatient and clinic
operations within the specialty. Excellent communication skills,
both oral and written, interpersonal, and facilitation skills.
Computer proficiency including Microsoft office suite programs.
Demonstrated ability to produce results and coordinate projects.
Proven critical thinking and problem-solving skills with the
ability to organize analyze and present
data.License/Certification/Registration RequirementsValid
California RN license *or* other professional license required.
Current BLS certifications Certified Psychiatric nurse, preferred.
Certified Clinical Nurse Educator/Leader, preferred.Ages of
Patients ServedThis position will serve Adult, and Older
Adults.Salary Range:72.20 - 108.30 USD HourlyThe Physical
