FTE 1 Scheduled Bi-Weekly Hours 80 Work Shift Day: 8 hours Job Description The Manager Post Acute Network develops, implements, and maintains an aligned network of post-acute care providers for critical services. The Manager establishes a network of quality, cost-effective services along the continuum of clinical care to prepare for value-based care programs, reduces readmissions from post-acute settings, increases acute care throughput by decreasing length of stay, improve patient experience. Establishes and reviews performance metrics to evaluate and track qualifying post acute care entities. The Manager establishes and maintains relationships with individual post acute entities. The Manager collaborates with care management services and collaborates with and supports service line continuum of care initiatives. Coordinates physician integration by organizing and supporting a physician advisory group to include primary care physicians, hospitalists, emergency department physicians, SNFists. The Manager also evaluates and implements processes to streamline post acute referrals within the post acute network. QUALIFICATIONS Bachelor's degree in Business Administration/Management. Master's degree in Business Administration/Management preferred. Two (2) years management or leadership experience preferred. Minimum five years of experience developing programs, providing information and referral to community services for older adults, and program management. Program development, planning, and coordination experience related to health or social services.6. Strong written and oral communication skills. Critical thinking, problem solving and decision- making skills. Strong organizational skills. Experience developing and maintaining relationships.10. Computer and internet proficiency. License/Certification/Registration Requirements Valid CA driving license and proof of auto insurance The Physical Requirements and Working Conditions of this job are available.
